  • [会议] Non-Intrusive Inspection of Pressure Vessels in HHC Service
    Pressure equipment has traditionally called for internal visual inspections augmented with applicable NDE to evaluate a pressure vessel suitability for continued service.This burden is extremely expensive and inherently unsafe.Recent recognized and generally acceptable good engineering practice (RAGAGEP) is allowing non-intrusive inspections (Nil),or external inspection in lieu of internal inspections,when the external nondestructive examination (NDE) provides data as good as or better than that provided by an internal inspection.Advanced NDE technology has progressed to the ability for providing extremely detailed information which,in many cases,is better than can be obtained by an internal visual inspection,and it is much safer.This paper will discuss and outline how to implement a non-intrusive inspection (Nil) program.Points to be presented and discussed include: 1. Advantages & disadvantages of an Nil program 2. Determination if an Nil should be performed alone or in conjunction with other inspection methods 3. Defining the requirements of an Nil 4. Selecting the appropriate Nil methods 5. Preparing the Nil plan 6. Performing the appropriate NDE inspections 7. Evaluating the results This paper will look at the high inherent risks associated with putting people into hazardous confined spaces versus the ability to fully assess a pressure vessel from the outside.The benefits of not entering confined spaces while still ensuring pressure vessel suitability for service are a significant risk reducer for any company.

  • [会议] The Roundabout Way to Disaster: Recognizing and Responding to Normalization of Deviance
    Humans develop processes to structure their activities to set and reach goals.Wherever a process exists,there will be always be deviations from that process.Skipped steps,workarounds,shortcuts,improvements,and innovations can be incorporated over time,gradually resulting in new practices that are accepted as the norm.Small changes that seem insignificant can easily be overlooked and therefore not managed.To the extent that individuals,teams,and organizations tolerate non-conformance,normalization of deviation can be established.The cumulative effect of these deviations may be unmitigated hazards that can lead to a catastrophic process safety event.A CCPS project subcommittee (Project 269) was established to develop a text to address the initial and long-term effects of normalization of deviations.The finished book,Recognizing and Responding to Normalization of Deviance,will be published in early 2018.This paper will provide examples of the depth and breadth of normalization of deviation throughout processes,procedures,and organizations,and will illustrate techniques to address its potential impact.

  • [会议] The influence of initial pressure on the explosion characteristics of 2,3,3,3-tetrafluoropropene
    Refrigerants are used more and more widespread,but there are potential safety hazards in the use of refrigerants.In particular,the possible explosion of refrigerants caused the world’s refrigeration industry attach great importance.In this paper,20L ball was used to study the explosive properties of refrigerant 2,3,3,3-tetrafluoropropene at different initial pressures.The experimental results show that the maximum explosion pressure increases with the increase of the pressure.When the pressure is constant,the maximum explosion pressure reaches thejnaximum value of 7.6%.In addition,this article also calculated the explosive index at various concentrations to indicate the size of the explosive power.

  • [会议] The VOCs Emission Source Intensity and Safety Protection Distance Analysis of Urban Refinery Based on QRA
    with the ‘urban refinery’ problem becoming increasingly prominent,the identification of VOCs emission source of urban refinery has been the problem to be solved mostly.In addition to the widely used monitoring methods,we combined with the wind tunnel experiment and numerical simulation technology to confirm the model coefficient,and established more universal and quantitative emission source intensity analysis model,with the goal to form a suitable assessment technology for domestic ‘urban refinery ‘VOCs emissions problem,as well as to provide scientific data support for the complete sets of environmental and security solution programs.

  • [会议] Case Study in Organizational Risk Profiling-Bridging the Gap Between Risk and Decision Making
    Many organizations today struggle with bridging the gap between the asset-level process safety risks known by the process safety professionals and operations leaders and the actions taken in relation to these risks by executives.Organizations have increasingly used tools such as risk registers and audit scores to shape management direction but too often these are not compelling enough on their own.In this paper,the author will share an experience working with multi-national high hazard manufacturing organizations in the creation of a corporate-wide risk profiling tool to enable more real-time and informed decision making by the corporate safety team and executives.The author will describe the selection process of inputs into the tool,such as kpi’s and existing work process outputs,in addition to new inputs and the impact of local plant culture on the results.Finally,the author will describe how this tool is used by organizations to drive higher levels of risk-based decision making and asset prioritization.
