  • [期刊] Health-Related Quality of Life and Sexual Function in Patients Treated for Penile Cancer

    Purpose: To assess the impact of primary surgery, partial and total penectomy, and of inguinal lymphadenectomy on health-related quality of life (HRQoL) and the sexual functionof penile cancer patients. Materials and Methods: We invited 15 patients surgically treated for penile cancer in the Regional Hospital of Specialized Medicine of the Yucatan Peninsula between 2012 and 2016 to complete the SF-36 quality of life and International Index of Erectile Function (IIEF-5) sexual function questionnaires. We evaluated the impact of primary surgery type and inguinal lymphadenectomy on the outcome of HRQoL and sexual function. Results: A total of 10 patients (66%) completed the SF-36 questionnaire. Surgery type and extent of resection were overall not associated significantly with worse outcomes on the assessed domains of quality of life. However, men treated with partial penectomy reported significantly more pain (p = 0.0547) than men treated with total penectomy. Similarly, men who underwent lymphadenectomy reported significantly more pain (p = 0.0547) than those who were spared from the procedure due to the primary tumor ' s stage and grade. Sexual function was severely affected (IIEF mean = 6.5) in patients treated with partial penectomy. Conclusions: Although partial penectomy and lymphadenectomy were significantly associated with more postoperative pain, no differences were observed in other HRQoL domains. Sexual function was severely affected in patients treated with partial penectomy.

  • [期刊] Effects of pH of processing-medium on re-dispersion of spray dried, crystalline nanoparticles of pure naproxen

    Crystalline nanodispersions of naproxen stabilized with hydroxypropyl cellulose were prepared at different pHs both below and above the pK(a) of the drug. After spray drying together with lactose, the measured particle size of the rehydrated dispersions using laser light scattering/Mie analysis was found to depend strongly on the pH of the original, processed nanodispersion. The pH-dependent solubility of this weak acid resulted in formation of a dissolved fraction of drug in the nanodispersions, this being higher as the pH increases. There was therefore a loss of crystalline naproxen during processing at higher pH, with fewer and/or smaller nanoparticles in the nanodispersion after nanomilling. After spray drying, the dissolved naproxen fraction formed an amorphous solid, as shown by differential scanning calorimetry and X-ray powder diffraction. The amorphous fraction redissolves on re-hydration. An improved disaggregation and smaller particle size of the nanoparticles at higher pH was observed because of the lower amount of crystalline nanoparticles now present. If the pH of the nanodispersion during processing is kept below the pK(a) for a weak acid, then a high yield of nanoparticles should be expected with little formation of an amorphous phase of the drug.

  • [期刊] Genetic engineering of sunflower (Helianthus annuus L.) for resistance to necrosis disease through deployment of the TSV coat protein gene

    Necrosis disease incited by tobacco streak virus (TSV) causes significant yield losses in sunflower in the tropics and sub-tropics. Genetic engineering of sunflower through deployment of the coat protein gene of TSV (TSV-CP) was attempted along with neomycin phosphotransferase gene (nptII) for selection of putative transformants on kanamycin. Split cotyledons from decoated mature seeds were used as target tissues for Agrobacterium tumefaciens-mediated transformation and putative transformed shoots were obtained with an average frequency of 3.3%. Presence of the introduced transgene was identified by PCR; expression was determined through RT-PCR, northern blot analysis and quantitative Real-time PCR (qRT-PCR); stable integration was ascertained through Southern blot analysis of plants in T-0 to T-4 generations. Out of 102 positive T-0 events, 20 events were carried to T-1 generation from which five events (CP-S-237, CP-S-247, CP-S-481, CP-S-648 and CP-S-753) were advanced till T-4 generation. Challenge inoculations of plants of event No 481 showed resistance to necrosis disease and plants grew to maturity in TSV-CP transgenics while control plants (untransformed) showed mortality within 1-2 weeks following inoculation. Expression analysis of the TSV-CP and nptII genes in different tissues at flowering and seed setting stages revealed constitutive expression of the transgene till seed maturation. One event (No 481) was selected for transfer of the TSV-CP gene into agronomically superior genotypes.

The SansEC technology is a proven wireless sensing platform capable of measuring the electrical impedance of physical matter in proximity to the sensor based on a change in its resonance response. The SansEC sensor also exhibits a unique characteristic to disperse the lightning strike current to help mitigate lightning damage. In a turbine blade application, an array of SansEC sensors will cover the surface area of the composite blade, providing both lightning mitigation and damage sensing. NASA Langley is seeking industrial partners/licensees to commercialize this technology. The research team at NASA Langley is available to assist with further development.

    PurposeTo investigate the prognostic impact of 4L lymph node (LN) dissection in left lung cancer and to analyze the relative risk factors for 4L LN metastasis.Patients and MethodsWe retrospectively collected data from 657 patients with primary left lung cancer who underwent surgical pulmonary resection from January 2005 to December 2009. One hundred thirty-nine patients underwent 4L LN dissection (4L(D+) group); the other 518 patients did not receive 4L LN dissection (4L(D-) group). Propensity score weighting was applied to reduce the effects of observed confounding between the two groups. Study end points were disease-free survival (DFS) and overall survival (OS).ResultsThe metastasis rate of station 4L was 20.9%, which was significantly higher than those of station 7 (14.0%; P = .048) and station 9 (9.8%; P < .001). Station 4L metastasis was associated with most other LN station metastases in univariate analysis, but only station 10 LN metastasis was an independent risk factor for 4L LN metastasis (odds ratio, 0.253; 95% CI, 0.109 to 0.588; P = .001) in multivariate logistic analysis. The 4L(D+) group had a significantly better survival than the 4L(D-) group (5-year DFS, 54.8% v 42.7%; P = .0376; 5-year OS, 58.9% v 47.2%; P = .0200). After allowing potential confounders in multivariate survival analysis, dissection of 4L LN retained its independent favorable effect on DFS (hazard ratio, 1.502; 95% CI, 1.159 to 1.947; P = .002) and OS (hazard ratio, 1.585; 95% CI, 1.222 to 2.057; P = .001). Propensity score weighting further confirmed that the 4L(D+) group had a more favorable DFS (P = .0014) and OS (P < .001) than the 4L(D-) group.ConclusionStation 4L LN involvement is not rare in left lung cancer, and dissection of the 4L LN station seems to be associated with a more favorable prognosis as compared with those who did not undergo this dissection.
